ABSTRACT

Objective:To compare the clinical application effect of polyetheretherketone (PEEK) and titanium in cranioplasty. Methods:Sixty patients with skull loss in Shengjing Hospital of China Medical University from January 2015 to December 2019 were retrospectively analyzed. Among them, 25 patients were repaired with PEEK material (PEEK group) and 35 patients were repaired with titanium mesh (titanium mesh group). The postoperative complications within 6 months, plastic surgery satisfaction, hospital stay and hospitalization cost were compared between 2 groups. Results:There were no significant differences in bleeding, infection, epilepsy, implant displacement, implant exposure, incidence of secondary operations and the length of hospital stay between 2 groups ( P>0.05). The incidence of subcutaneous fluid, good plastic surgery satisfaction and hospitalization costs in the PEEK group were significantly higher than those in titanium mesh group: 28.0% (7/25) vs. 5.7% (2/35), 88.0% (22/25) vs. 62.9% (22/35) and (12.4 ± 1.5) × 10 4 yuan vs. (7.0 ± 2.4) × 10 4 yuan, and there were statistical differences ( P<0.05 or<0.01). Conclusions:PEEK material is safe and effective in cranioplasty, and the treatment satisfaction is better, but the cost is higher. PEEK can be used as an ideal skull repair material for patients with economic basis.

ABSTRACT

Objective:To explore the application effect of 3D printing technology in comprehensive reconstruction of thumb and finger. Methods:From January, 2018 to January, 2020, 67 patients with 84 thumbs and fingers defects were selected, which were 37 thumbs and 47 fingers, and 45 of I-III degree and 39 of IV-VI degree. The method of operation was comprehensive reconstruction of thumbs and fingers assisted by 3D printing technology. The patient's hands and feet were scanned with CT and 3D modeling, and the 1∶1 model was derived. The skin model and bone model of thumb and finger defect were printed. Put the skin model on the great toes, and design the shape of the nail flap to be cut. Then the flap was spread in the groin area to design the perforating branch flap of the superficial circumflex iliac artery. The interphalangeal joint or metatarsophalangeal joint of the second toe was cut off according to the skeleton model (when the length of the bone joint was not enough, the iliac bone strip was taken to be connected in series). The effect of operation was observed in outpatient. Results:All of the 84 thumbs and fingers survived and were followed-up for 3 to 24 months. The appearance of the reconstructed thumbs and fingers was similar to that of the normal ones, with good texture and elasticity. The nail was smooth and glossy. The sensory recovery was S 3+, and the TPD was 4-6 mm. According to the Evaluation Standard of Finger Replantation and Reconstruction of Hand Surgery Society of Chinese Medical Association, 70 thumbs and fingers were excellent and 14 thumbs and fingers were good. In 64 thumbs and fingers, all toes were preserved in the donor area, and the color and texture of the flap were satisfactory, which did not affect walking, running and jumping, and had no walking pain. Conclusion:With the help of 3D printing technology, the tissues can be cut more accurately during the thumb and finger comprehensive reconstruction, which not only improves the beauty of reconstructed thumbs and fingers, but also avoids unnecessary trauma and improves the satisfaction of patients.

ABSTRACT

Objective To understand the phenotype and enzyme genotype of pan‐drug resistant carbapenemase‐producing Acine‐tobacter baumannii to provide the evidence for clinical rational use of antibiotics and monitoring hospital infection .Methods A total of 117 clinically isolated strains of Acinetobacter baumannii were collected and performed the routine microbiological detection . Multi‐drug resistant Acinetobacter baumannii was screened by K‐B disk diffusion method .The phenotype of carbapenemase‐produ‐cing strains was detected by using the Carba NP colorimetry and modified Hodge test .The drug resistant genotype of multi‐drug re‐sistant Acinetobacter baumannii was verified by PCR .Results Among clinically isolated 117 strains of Acinetobacter baumannii ,64 strains were multi‐drug resistant Acinetobacter baumannii ,in which 33 strains were carbapenemase positive .OXA‐23 drug‐resistant genotype of carbapenemase was detected by PCR ,while IMP ,VIM and NDM‐1 drug resistant genes were not detected .Conclusion The CarbaNP method can rapidly detect carbapenemase‐producing strains with the advantages of strong sensitivity and simple oper‐ation ,which conduces to improve the detection rate of carbapenemase‐producing strains and monitor the nosocomial infection .

ABSTRACT

Being an important basic medical course, Pathogenic biology is a bridge discipline to connect basic medicine with clinical medicine. Experiment teaching of pathogenic biology has auxiliary function for grasping and understanding the theory of knowledge. As a new teaching mode, microlecture brings more changes for classroom teaching. Microlecture video has a positive role in arousing students’learning interest because of some characteristics including short, vivid, visual and auditory coordination. This article disccussed the advantages of teaching ideas and methods of microlecture, combined with experimental course contents and features of pathogenic biology to introduce the application and function of microlecture in experimental teaching of pathogenic biology.
